Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.
Option 1) Payment at time of service. All treatment paid for in full with cash or check at the time of service will receive a 5% bookkeeping discount. We can pass this 5% savings on to you because prompt payment saves our office the cost of processing your bill.
Option 2) Visa or Mastercard payment
Option 3) Care Credit Health Card This program allows our patients to establish a credit relationship with a third party. This option allows us to proceed with your dental care without the worry of having to make payments to our office at every visit.

Dental Insurance
Dental insurance is intended to cover a portion, but not all, of the cost of your dental care. Most plans include some type of co-payment, a deductible, and other types of expenses that must be paid by you: the patient. Most plans also have an annual maximum amount of benefits that will be paid, after which no treatment will be covered.
We will work with you to assure that you receive the maximum benefits to which you are entitled. As a courtesy to our patients, at the end of each visit all work completed will be submitted to your insurance carrier for you. Please remember that it is your responsibility to see that your account is paid, regardless of your insurance coverage. Your insurance contract is a contract between you and your insurance carrier.
